The components of implementing evidence based practice project consist of making a decision on how to present and educate all members of the interdisciplinary team, patients and caregivers. One of the important results obtained in our study was that the lack of physician recommendation was among the leading reasons for not getting a vaccination (Korkmaz et al. 2019). The education will consist of for all individuals patients and staff videos, literature, brochures, data posted for all to view, and questionnaire. Phase four implementation, once October begins all patient will be ask monthly duirng the time frame about receiving the vaccine until it is administred. Also data for vaccine administration, hospitalization and missed treatment due to influenza and pneumonia will be track and added to monthly quality data. The most difficult piece is the educating the interdisciplinary team. Social work and dietary services feels as if encourage vaccine administration is a clinical aspect and they do not feel comfortable addressing the questions that may arise. The piece that has gone well is the medical director has embrace the project with open arms and want to incorporate into practice as a quality measure. A piece of advice I would recommend about the implementation process would be to ask for assistance do not do the task alone.

POST 2

The components associated with this writers project involve creation of the written tools (LACE Scores) for assessing patients that are high risk for admission, educating multi-disciplinary staff members and having them complete their individual patient education, making follow up appointments, coordinating pharmacy delivery to the bedside and follow up phone calls. The most difficult part of this implementation has been coordinating multi-disciplinary education due to time constraints. For example, the pharmacist has the time to do medication education but the patient is off the floor for testing or the pharmacist is busy and has not had the ability to come to the floor by the time the patient has been discharged. Another problem with the education portion of this implementation is the amount of enhanced education versus the patient or caregivers ability to retain what is being taught, particularly if this is a new diagnosis of heart failure. Lippincott Solutions published 5 strategies that nurses, and other educators can use to help patients retain the information that is taught to them. They are as follows:

Take advantage of technology. Consider creating power point presentation or videos. If using paper for patients to read and refer to, customize them to the patients unique situation
Understand how the patient learns. Education can be provided in many ways and can be delivered to best meet the patients needs. Teach back is a good method
Make the patient understand why this education is so important
Consider the patients strengths and limitations
Include family caregivers as part of the teaching process

The commitment from the nursing staff has been strong. Everyone is engaged and eager make this project successful and nurse engagement correlates directly with critical safety, quality and patient experience and outcomes. (Dempsey, 2016)

This writers advice would be to start sharing information about the goal at the very beginning, when the PICOT questions is developed. Ask for multidisciplinary input prior to developing interventions and even before the literature review. Make the staff that is doing the work part of the project.
