Select a client with mental health and substance abuse issues from interaction at your field placement, and write three progress notes for interactions with that client (in SOAP, PIE, or DAP format, whichever you prefer).  If your placement is not the type for multiple interactions with the same client, you may write one note for three different clients.  You must be the one having the client interaction (not an observation of someone else’s interaction with a client), and the client may be at any system level (micro, mezzo, or macro) which is served by your field agency. 

To protect the confidentiality of your client, please remember to change the name and any specific identifying information when writing your case notes.  Note that three strong case notes are required for this assignment.
