Evaluate data to identify trends across the healthcare spectrum.

Scenario

You were recently hired at a healthcare organization to identify negative trends related to patient care and devise quality improvement initiatives. Since you only recently started working at this healthcare organization, you begin examining data to find processes related to patient care that are not working efficiently to find and track solutions. After determining a process you are going to improve, you plan to write  report about the improvement initiative and design a reportable dashboard to provide to the executive board of the healthcare organization.    

Instructions

Part One: Complete  report including the following information: 

Identify the process related to patient care you have selected. Summarize the steps of the process and identify the portion of the process that needs to be improved.

Establish a quality improvement initiative for the process.

Develop a SMART Goal for the initiative.

Establish a project timeline and reporting schedule.

Determine the data elements needed to track results of the initiative including how the data will contribute to improving patient care.

Explain how the process can be maintained after the quality improvement project ends.

Must provide external references that support your quality improvement initiative.

Part Two: Using imitation data, design a reportable dashboard within Excel that shows the data elements that need to be collected to track the project. The dashboard must include all sections of the project (benchmarks).
