• Give a loading dose of 40 mg furosemide in 50 mL of Normal Saline intravenous piggyback

(IVPB) and infuse over 15 minutes:

Initiate at 20 mg/hr IV furosemide continuous infusion using a concentration of 200 mg Furosemide per 100 ml 

• If urine output is less than 1 ml/kg/hr increase the furosemide infusion rate by 10 mg/hr as needed to a max dose of 60 mg/hr Furosemide Vials contain: 40 mg/4 mL
