Introduction (Home Page) (100-150 words)

This is the first page people will see when they arrive at your site. It is an opportunity for people to get to know you and what you’re about. In creating the introduction, think about the connections that run in your professional and, perhaps, personal life. Are you focused on one issue? Are you more interested in strategies and policies? Does a theoretical foundation drive you? This section should be brief but descriptive. It should set the tone for your site and provide users with a quick view into the content/philosophy of your public health work.

Professional Mission and Vision Statement (50-75 words)

This mission and vision statement should be concise and direct and should be connected to your introduction in some manner.

Resume

In this section, copy in and/or attach your resume and feel free to include any relevant information you want to emphasize, such as contact details, awards, and so forth. You should include but are not limited to selected academic papers and presentations, civic engagement activities, and personal strengths and abilities. You may also want to link directly to your Linked-in profile, or any other professional sites.

Competencies in Public Health aligned to APE Products

CEPH MPH Foundational Competencies

Upload evidence of assessments from your curriculum for each of the 22 CEPH MPH foundational competencies identified in the table (Appendix A) aligned to demonstrate achievement. Be sure to include a Reflective Statement (50 – 100 words) for each artifact on your own understanding of the competency domain – what it is, how it’s used, and its role/value in the field. The statement should illustrate your personal understanding of a competency and perhaps your deeper philosophy of that competency.

CU MPH Concentration Competencies

Upload evidence of assessments from your curriculum for each of the five CU MPH concentration competencies identified in the table (Appendix B) aligned to demonstrate achievement. Be sure to include a Reflective Statement (50 – 100 words) for each artifact on your own understanding of the competency domain – what it is, how it’s used, and its role/value in the field. The statement should illustrate your personal understanding of a competency and perhaps your deeper philosophy of that competency.

ILE High-Quality Written Product

Present an overview, two or three sentences that describe the scope or scenario which represents the high-quality written product. Then, upload copies of the assessments as evidence of competency achievement.

Upload a copy of the completed program plan or other written product developed which demonstrates synthesis of learning.

Upload a copy of the ILE Poster Presentation

Applied Practice Experience Projects

Upload a copy of the following artifacts in this section:

CITI Training Certificate

Student Learning Agreement

APE Hours Log

Evidence of TWO projects/products,

Upload evidence of the two (2) products from your Applied Practice Experience coursework to highlight within your portfolio and specify the competencies each represents.(Remember your two products must represent at least 5 competencies of which 3 must be foundational and the others must represent the Chamberlain University MPH-generalist concentration competencies).

Be sure to explicitly connect your evidence to your field work and student learning agreement. That is, describe how the project is connected to show the integration of practice. Be sure to include all relevant documents in your evidence sections. You will attach these documents so that they will appear at the bottom or sidebar of each competency.

For each of the two products developed during APE, there are two main sections within each of the five selected competencies that you will need to compose: “Reflective Statement” (about the competency) and “Evidence of Competence”.

Reflective Statement (50-100 words)

The reflective statement should be a reflection on your own understanding of a particular competency domain—what it is, how it’s used, and its role/value in the field. The statement should illustrate your personal understanding of a competency and perhaps your deeper philosophy of that competency. For instance, if community participation drives your program planning work, use this space as an opportunity to describe that belief, how it applies to program planning and what its value is to public health. Keep in mind that this section can and in fact should be brief. A single very well written paragraph is fine.

Evidence of Competence (150-300 words)

Use this section to connect your reflection to your practical experience in the competency. Write a brief description (or executive summary) of work you have done in an area (community assessment, program planning, and so on). This section should describe each project in brief form. When appropriate, the evidence of competence section should include the following headings:

Background/Overview of Project related to achieved competency

Methods

Findings

Recommendations

Next Steps (if applicable): For your community assessment, this section could mention the program plan that was developed based upon the assessment, with a link to program plan section.
