1.   Define Transition and discuss why it is so important that Congress enacted legislation requiring it be addressed in the IEP.

You can use the language in IDEA or another document, but be sure to cite the source.

Discuss how the implementation of transition services has evolved since PL 94-142. Include information on federal and state requirements for transition planning, age of implementation, who is involved, their responsibilities, a list of possible coordinated activities, what is a plan of study, explain services offered by DRS, explain age of majority and a timeline of services.

This section will need to be informative and easily understood.

2.    Develop a list of approved district transition assessments and provide a detailed description.

Give specific examples of formal and informal transition assessment instruments used by YOUR district and explain their significance. You may need to talk to a secondary SPED teacher or your SPED director who can help you identify assessments used by YOUR district. If this proves to be too difficult, compile a list of formal and informal assessments you think beneficial and appropriate. This list needs to be a minimum of 6 assessments.

You will also need to include additional appropriate formal and informal assessments. Use the resources presented in the presentations or other sources.

I am in the Tulsa Public School District in Oklahoma also I have included a couple of assessments, however they are not used in my district. if you end up coming up with own assessment please use 3 formal and 3 informal transition assessment
