An effective team is recognized internationally as one of the best ways to achieve better patient outcomes. It promotes better healthcare delivery and is patient-centered. Team-based health care is the provision of adequate or appropriate healthcare services to communities, individuals, or families by at least two healthcare providers (Reiss-Brennan,2016). Healthcare providers work in collaboration with patients and their caregivers to ensure better outcomes among patients. Team-based care is usually done to attain healthcare objectives and provide better treatment to the patients.

In healthcare organizations, effective team-based care is one where team members, including the patients, communicate adequately to improve healthcare provision. This allows healthcare professionals to merge their expertise and observation methods so that patients are administered with the best healthcare treatment (Amir Babiker, 2014). Team-based care also requires healthcare providers to understand the culture and medical history of their patients. It allows them to identify the best ways of administering medication to different patients. Team-based care in healthcare organizations has also significantly improved over time. Nowadays, patients are rarely taken care of by just one professional. A team of healthcare professionals looks after a single patient; an effective team is necessary to ensure patient safety (Amir Babiker, 2014). It is also essential since it minimizes errors or miscommunication primarily because the healthcare providers are skilled differently. Therefore, treatment is done better than when only a single healthcare provider takes care of a patient. Consequently, patients are interested in their health. Team based-care today allows patients to be part of the communication process. The involvement promotes transparency in treatment; it also ensures better patient outcomes hence satisfaction in administering care.
