Describe how your improvement aligns with the Triple Aim (better care, better health, lower cost).
Apply Benchmarking (1-2 slides)

Key Performance Indicators (KPIs) are measurable values used to assess how effectively an organization, unit, or process is achieving key objectives. In healthcare, benchmarking or KPIs help track performance over time and guide data-driven improvement.
Explain how benchmarking (comparing your organization’s KPIs to internal or external standards) helps you understand current performance and set measurable improvement goals.
Reference at least one benchmark source, such as:
CMS Quality Net, National Database of Nursing Quality Indicators (NDNQI), APQC, or internal institutional data.
Include one example KPI or metric relevant to your practice problem.
Apply the PDSA Model – “Plan” Phase (2–3 slides)Using the IHI PDSA Worksheet as your guide, outline the Plan phase of your improvement process. Include:

AIM statement: What are you trying to accomplish?
Proposed change: What improvement or intervention will you test?
Prediction: What do you expect to happen and why?
Measures: What outcome, process, or balancing metrics (KPIs) will you track to evaluate change?
