This week you are submitting the design of your database to Patton-Fuller leadership for approval. The objective of this project was to enable you to understand better the factors involved in health information architecture, design, and management to support decision making in your professional role that lies ahead.

The design phase of this project culminates in a presentation to Patton-Fuller stakeholders. As you present your database design, the objective is to “sell” the plan to enable the project to move forward into development.

Prepare a 1,400- to 1,750-word report that includes the following sections:

• Problems and Solutions: Explain the objective of the database project, including the business requirements you sought to fulfill by designing a new health information database.
• Use Cases: Describe who would benefit from this resource and how they would use it. Specify the roles and why each role needs the information that could be collected and sourced from this database.
• Database Model: Summarize the overall data architecture. Explain data table relationships and how this data is collected. Include the ERD diagram to illustrate these relationships.
• Data Collection and Reporting: Describe the various forms and reports you created to collect data. Detail who would be using the forms (external individuals and/or internal roles) and who would be using the reports (internal roles).
• Expected Outcomes: Explain how this database will serve the organization to improve workflows and communication for internal personnel, patient outcomes, and the experience of partner organizations.
• Summary: Include a summary of all points.

Cite at least 3 reputable references. One reference must be your textbook, Concepts of Database Management. Reputable references include trade or industry publications, government or agency websites, scholarly works, or other sources of similar quality.
