For this assignment you will create a PowerPoint presentation including speaker notes.

You will develop a case presentation that focuses on the pharmaceutical regimen for a patient who has two primary conditions 1) cardiovascular condition of your choice based on the readings and 2) hyperlipidemia. You will construct the presentation as if for a group of your professional medical provider peers. The premise is that the advised non-prescription and prescription regimen has resulted in improved outcomes. Thus you must show initial patient labs or measures, your interventions and prescription changes and the follow up outcomes and measures. Include the examination of effective communication and shared decision-making modalities for the patient and family. Address any determinants of health and how this may affect the compliance and patient outcomes, etc. Interprofessional collaboration regarding pharmaceuticals is included. Content on applicable determinants of health as they relate to the patient are included. This is a fictional patient.
