TITLE OF YOUR PAPER2Working at PCCS has benefited me in so many ways. For starters it made meunderstand the actual and correct definitions of disabilities. To me, the word disabilitymeant someone who was handicapped or who suffered from autism at the beginning.However, it’s so much more than that, a person who has a diagnosis is a disability. Suchas dyslexia, ADHD, andAlzheimer’s disease. Arrhythmia.Arthritis, chronic kidneydisease (CKD), diabetes, etc. one can never find out who has a disability based onappearance so easily. It has been an absolute pleasure to meet wonderful peoplethroughout PCCS who have welcomed me with open arms. I also got to work with thepeople we support one to one which made me get to know them on a personal level. Amember I’ve been working with has a very special story that I’ll be discussing, but I can’tdiscuss names because it’s confidently sensitive.

He currentlylives on his own in his apartment, andhe is his ownadvocate, he has Dayhab in PersonCenterd Care Service PCCS five days a week but he is not Constance with those days.My supervisor and I are working with him to at least come in 3 days a week and havehim participate and become active more with the program and work with him to meethis goals. This person is not close with his family at the moment. He was only closewith his older brother who passed away last year which made things very hard for himmentally because he lost the only family that he truly saw as family. One can onlyimagine and feel his pain. This individual is very known thought it his community but notalways in the best way possible because he gets into many gang violence, street drugs,etc. Additional,y these violations has to lead him to go to jail in the past and are still inprocole and we are working on how to show him how to make better choices for hisown safety and others’.Assessment of the practice situation, and interventions.My assessment practice with this client is to work on his goals one to one, creatinghis staff action plans and completing them, and discussing them with his case managerduring his 6-month meeting. I get to work with him for about 45 minutes on the two daysthat I am there we are currently working on identifying different types of emotions andhow to deal with each emotion with coping skills. I would also not only go back with my
TITLE OF YOUR PAPER4supervisor but I would have meetings once a week with aDeputy Executive Directorwho works in this nonprofit agency where she helps guides me with my decision-makingwith my clients.The effectiveness of service/interventionsBased on my equations of services and interventions I have definitely seenprogress not only with working in this agency but the people that I have worked with.Even though it was little time but I have seen improvement with my client. Such as, Ihelped him make some calls for AA which can help with his drug, and violence problemsaddiction to join this program to stay out of trouble and more importantly to keephimself and others safe at all times. Additionally, we have created a chart where he putsdown how he is feeling and why and what he feels like doing when he is feeling thatway. Based on the responses we have different coping skills that had worked very wellwith him. Such as, when he is feeling angry he and a staff member (DSPS) or myself getto go to a quiet room and focus on his beratings in and out about 6 times then he wouldput on headphones where he gets to count backward to 20 out loud. We tried multiplecoping skills such as taking walks, sensory toys, etc., but this is the one that worked wellwith him so far. In the end, I and the staff would talk with him and explain the differentways this could’ve turned out if he did not use this coping skill which kept him calm andmade very healthy choices.A description of any factors in the relationship between you and the clientsystem that had an impact on the service
TITLE OF YOUR PAPER5In the beginning, this client was refusing any help from me or the supervisoras he explained it he wants to live life the way he wants to and that he doesn’tcare what anyone thinks or does. This made him in a very difficult spot where Iand my supervisor explained to him if we do not meet halfway by meeting theDayhab requirements there is a huge risk that he can get disenrolled and kickedout and he will be losing all kinds of the services that he receives. This was veryhard because no one ever wanted to see this person go because he is reallygreat he is very thoughtful, strong, and incredibly smart, and he always knewhow to make people smile at the end of the day. I had built a very fast bond and Iknow he is cable of a great an amazing future but really needed people not togive up on him and support him in the most possible way that we can.SummaryGetting to build relationships and bonds with the people at person-centeredservices (PCCS) has impacted me in a really significant way. I never imaginedthat I would truly enjoy this and educate myself with this nonprofit organization. Iwouldn’t have been if it wasn’t for my supervisor, Staff members (DSP), and thepeople that we support and worked with. I am truly happy and proud to see theprocess that one of the people we support is making and yet to see what’s tocome.
