HarrietObservation— Dishes October 3, 2022, at 3:56 PM S: Client did not communicate any relevant comments during treatment, but acknowledgedstatements made by the therapist by responding with phrases such as “okay,” “oh,” and "yepf’ 0: Client participated in 3-minute OT session at home kitchen sink for skilled instruction in dishwashing techniques while standing. Client used walker and moderate assistance to ambulate from doorway to sink. While standing at the sink, the client used propping and minimal assistance to stabilize and align her body during the task. Client demonstrated ability to stand for duration of the task without needing a break. The client needed 0 verbal cues to initiate the task of turning on the faucet to fill the pot with water, however, she needed minimal verbal cues to initiate and organize the task of washing the dishesand utensils with the sponge. The client required total assistancein grasping and reaching with the left upper extremity. The therapist provided this support by using hand over hand assistance. The client attended to the task and maintained focus until she finished. She exhibited a high frustration tolerance when she encountered difficulties with the task. The client responded to verbal instructions and questions with correct responses 1-"? times and her responses were spoken fluently and clearly. The client exhibited ability to follow directionsAt the completion of the task, the client was able to make and establish eye contact with the therapist and acknowledge that the task was complete by noticing that there were no more dishes to be cleaned. hello, lam an OTA student. This is the subjective and objective documentation from a client ( her name is Harriet, whos 80 Lip) vvhos left hand lower extremity doesnt work properly because of OVA. The practioner was helping her doing the dishes and washing the utensils. I am trying to give an idea. since i cant post the video.
