Identify the purpose of ethical codes for health care providers This Week’s Detailed Case Study Information An ethical dilemma (ethical paradox or moral dilemma) is a problem in the decision-making process between two possible options, neither of which is acceptable from an ethical perspective. The Hippocratic Oath, an oath historically taken by physicians and other healthcare professionals, implies that healthcare professionals will make choices that benefit patients and will protect others from harm. However, medical care decisions often represent a conflict between what the health professional considers is in the best interest of the patient and respect for the patient’s wishes. Care decisions can also involve a conflict between the needs of the individual and the organization or between the organization and society. Please review the following 2 case studies regarding the ethical dilemma: Case 1: Everly, a 52-year old woman with a history of asthma, chronic obstructive pulmonary disease, hepatitis C and substance abuse, presented to the emergency department after cardiac arrest at home with subsequent return of spontaneous circulation. When Everly’s husband didn’t respond to attempts to contact him, with no advance directive, it was deemed more helpful to support the care team’s communication efforts and to provide an ethics perspective with procedural recommendations and "what if" options. Case 2: Bella is a 79-year-old woman who had suffered from hypertension, diabetes, and many strokes. She had an advance directive, which indicated that she did not desire any surgery, antibiotics, resuscitation, or tube feeding. When it becomes very difficult for her to eat, requiring an aide for a couple of hours each meal, her daughter decides that it is appropriate for a feeding tube to be placed as Bella does not have a fatal illness, just simply requires adequate nutrition.
