The Personal Reflection Paper has two main objectives: Physical Therapy Internship

1. To create a document written in a narrative format that reflects how your internship experience in Physical Therapy has provided an educational experience.

2. To create a document written in a narrative format that tells your “story” of what the Physical Therapy Internship experience has taught you from a philosophical and scientific perspective.

The paper is graded using the following criteria:

Each of the following bullet points must be used as a paragraph title in which the subject is addressed based on the internship experience. The paragraph subtitle must be in bold print and underlined. The paper must be a minimum of 5 pages long (not including your title page), double spaced, and typed in a 12pt font.

Your Reflection Should include the following:

1. Title page

Titled: Internship Reflection

Internship Site Name

Internship Site Supervisor

Dates of the internship

Student’s Name

2. Introduction

Give a precise overview of the internship with clear goals and objectives set for your internship experience.

Goals and Objectives must be specific, measurable, attainable, reasonable, time-sensitive.

3. Objectives

Detail your original objectives that you tried to accomplish during your internship experience

Detail your adaptive Objectives. These are unplanned changes that you had to make during your internship experience. This could be due to a change in duties, a change in personnel, or growth opportunities that you were not expecting. Explain how you adapted to still meet your objectives and successfully accomplish your internship.

4. History (selection of site, duties, and responsibilities)

Give a history of the organization/company that you are working with

Explain how you chose this site for your internship & why you chose this site

Include your duties and responsibilities at your internship site and contributions you made to the company during your time there.

5. Discussion

What lessons did you learn during this experience?

What educational contradictions did you encounter?

What sources of research did you need to utilize to successfully complete your internship?

How did you improve upon your communication skills to successfully complete your internship?

6. Conclusion (what did you learn, personal Reflection)

Provide a clear & concise summary of your internship experience

What did you learn during your time in the internship?

How has this experience prepared you for achieving your career goals?

Give a personal reflection of your growth over the last 15 weeks, professionally and personally.

7. Recommendations

Was your selection of this internship a wise choice for you, why or why not?

What future improvements would you suggest to the internship site for the next student?

What recommendations would you give yourself to improve on the goals and objectives you set for yourself at the beginning of your internship experience?
