As you establish your goals and objectives for this course, you are committing to an organized plan that will frame your practicum experience in a clinical setting, including planned activities, assessment, and achievement of defined outcomes. In particular, your plan must address the categories of clinical reasoning, quality in your clinical specialty, and interpersonal collaborative practice.

For this Assignment, you will consider the areas you aim to focus on to gain practical experience as an advanced practice nurse. Then, you will develop a Practicum Experience Plan (PEP) containing the objectives you will fulfill in order to achieve your aims. In this practicum experience, when developing your goals and objectives, be sure to keep PMHNP clinical skills in mind.

To Prepare

• Review your Clinical Skills Self-Assessment Form you submitted last week, and think about areas for which you would like to gain application-level experience and/or continued growth as an advanced practice nurse. How can your experiences in the practicum help you achieve these aims? There may be overlap between your skills goals and your PEP goals.

• Review the information related to developing objectives provided in this week’s Learning Resources. Your practicum learning objectives that you want to achieve during your practicum experience must be:

o Specific 

o Measurable 

o Attainable 

o Results-focused 

o Time-bound

o Reflective of the higher-order domains of Bloom’s taxonomy (i.e., application level and above) 

• Discuss your professional aims and your proposed practicum objectives with your Preceptor to ascertain if the necessary resources are available at your practicum site.

• Select one nursing theory and one counseling theory to best guide your clinical practice. Explain why you selected these theories. Support your approach with evidence-based literature.

• Create a timeline of practicum activities that demonstrates how you plan to meet these goals and objectives based on your practicum requirements.

Estimate how many hours you expect to work on your Practicum each week. *Note: All of your hours and activities must be supervised by your Preceptor and completed onsite. Your Preceptor will approve all hours, but your activities will be approved by both your Preceptor and Instructor. Any changes to this plan must be approved.

I also understand that I must see at least 80 patients during my practicum experience. I understand that I may not complete my practicum hours sooner than 8 weeks. Summary of strengths

My studies at Walden University have compelled me at work to challenge myself even though am not an NP yet. I collaborate with the residents, psychiatrist and other health care workers, including para-legal in the psychiatry Center I work. During a client’s intake/admission, I ask client questions, which helps me identify and recognize the symptoms the patient/client is having. Like Ryan Tanap (2017) claims, recognizing a mental patient’s symptoms become important to achieving the right diagnoses. This art I have mastered. Technology has made things easy, as am able to research quickly and select evidence based clinical guidelines that will help with each patient’s psychotherapeutic plan, Schutte-Rodin S, et al (2008)’s statement relating to individualizing In addition, performing and interpreting family history as it affects a patient is salient in developing a list of differential diagnosis leading to the formulation of a patient’s diagnosis, proficiency in this has come with experience motivated by the desire to excel. Moreover, at work I collaborate at all time with different experienced psychiatrist who has given confidence. They claim, my clinical practice in mental health settings has given me the knack to identify possible diagnosis for the signs and symptoms for individuals we take care as well as having non-judgmental practice approach and empathy while making sure I put into consideration ethical and legal problems at bay. And now at this practicum with one of the psychiatrists (Delaware Psychiatrist Center) and one of Maryland State Community Wellness Centers, I hope to demonstrate the application of age-appropriate therapeutic counselling and treatment of mental disorders across lifespan.

 

 

 

 

 

 

 

 

Opportunities for growth:

I would like to be proficient in selecting the correct screening instrument appropriate for any clinical situation. I took notice each facility makes do with a particular screening instrument they have chosen. It will be nice to broaden my knowledge with the implementation and interpretation of the screening instrument efficiently, as this will help in the appropriate diagnosis and care plan of the patients I see. Furthermore, increasing my knowledge in the area of knowing the right psychiatric recommendations and consultation when the need arises will improve my expertise as a PMHNP. Am told the continuous use of the DSM-5 will improve my diagnosis of the patients I will seeing on this practicum journey, by identifying each patient’s signs and symptoms.

1. Goal: To be opened for criticisms from resident doctors, psychologists, other allied workers, as well as the psychiatrists.

2. Goal: Effectively select correct screening instruments appropriate for diagnosing a clinical problem implement the screening instruments and interpret results.

a. Objective: Efficient interpretations of screening instruments are instrumental to correct diagnosis.

b. Objective: This will facilitate proper and appropriate plan of care.

c. Objective: This will also boost my confidence when relating with patients.

 

3. Goal: Sorting out the appropriate evidence based clinical practice guidelines for medication plan

a. Objective: This will prevent any adverse effects of any medications I will be prescribing.

b. Objective: It will prevent malpractice.

c. Objective: Planning will enable better monitoring of my clients

 

4. Goal: Selecting effective, safe and cost effective, individualized for the patient.

a. Objective: This will make me adhere to national guidelines and local formularies.

b. Objective: Treatment and Care for mental illness is so far trial-and-error, being cautious will be my uttermost in caring for my patient.
